The SCR is an electronic record created and maintained by the NHS. It contains key health information such as your current medications, allergies, and any adverse reactions to medicines.
Authorised members of our clinical team may view your SCR when it is relevant to your care. Access is strictly controlled and logged. We will normally ask for your permission before viewing your SCR unless there is an emergency and you are unable to give consent.

Devon & Cornwall Care Record (DCCR)
The DCCR is a local shared care record that allows health and social care professionals across Devon and Cornwall to securely view relevant information about you from different organisations involved in your care. This may include details such as diagnoses, test results, care plans, and referrals.
Access to the DCCR is strictly controlled, audited, and limited to professionals directly involved in your care. Viewing this information helps improve coordination between services, reduces duplication, and supports more timely and informed decision‑making.
