Making a Referral

We accept referrals for NHS patients.

Please follow your local protocol regarding referrals – we accept direct referrals by secure email, fax and post. Please call us if you would like any further information regarding making a referral to SpaMedica.

If referring via Webstar or OptoManager please select SpaMedica from drop down field – if SpaMedica is not showing then please state SpaMedica as service provider.

Where the e-referral service is not available, we accept written referrals by secure email and fax and post, as below:

Referral Form

Your referral form should include:

  • SpaMedica hospital location
  • SpaMedica free transport if required

    for patients who are mobile, require limited assistance and live over 10 miles from local SpaMedica hospital

  • For SpaMedica Accredited Optometrists –

    please confirm that you will be performing the post-op assessment
