Pre-operative assessment - What do I need to bring with me to my appointment?

For your cataract pre-operative assessment, please bring: 

• Your completed health questionnaire (sent with your appointment letter)
• Your current glasses, if you wear them
• A list of all medications you take
• Contact details for the person picking you up, if they won’t be waiting at reception
• Diabetic record book, if applicable
• INR record book (yellow book) if taking warfarin
• Consultant details if you have glaucoma 

 Soft contact lenses should be removed at least 48 hours before your appointment and hard lenses at least two weeks prior.
